AVEC now offers interactive forms that you can fill out online and print off on your home printer before coming to your appointment.

You may use the "tab" buttom to move from field to field as you type. When you have completed the form click the "Print" button in the upper right hand corner of the form to print it.

Once you have successfully printed it click the "Clear Form" button to delete all of the forms information.

To fill out the forms in your web browser just click the links below. If you wish to download a copy of the form to your computer then "right click" on the links below and choose "Save Linked File as..."

Please make sure you complete all three forms.
